Ad Nickname - Make up a short, descriptive nickname for the ad, for example: "Cold Latina". This helps us to identify the ad you saw from thousands of others. It's OK if you're not sure of the exact name.

Location of Ad - Please specificy where you saw or heard the ad:
- If you saw the ad on TV, then please tell us what show you were watching, and on what local TV station (please provide call letters).
- If the Ad was in a magazine, please tell us the name of the publication and in which issue the ad appeared.

Product - Our lists of alcohol products are by no means comprehensive. If you wanted to Talk Back about an Ad for a product not listed, please tell us at:

Date seen - Particularly important for print ads (newspaper or magazine) to provide date of publication.
Aproximate time seen - most important for broadcast ads. Please provide local times.

Your name: The Marin Institute will never disclose or sell your information to another organization! We need your personal information because the alcohol industry trade organizations will take your concerns more seriously if you provide a full name and mailing address with your Talk Back complaint. This information also allows them to identify your media market which can help pinpoint exactly which ad you saw.
